As regards the lower grades of coal the despatches were very limited owing to acute shortage of wagons and the large stocks at the end of the year were mainly coal of the poorer qualities. [...] From the fact that the props were not broken by the weight of the roof but swayed out it appears probable that the props wore not correctly set in relation to the dip and several props on the edge of the fall were afterwards found to be incorrectly set and liable to be swayed out by horizontal movement of the roof. [...] A ventilating fan is fixed at the shaft and with the object of inducing a current of air through the workings of the mine the top of the shaft is completely covered. [...] The jarring effect caused by the lifting of the -wooden shutters at the top of the shaft on the completion of each wind may have brought about the crystalline condition of the metal of which the shackle is composed The shackle appears to have been in use for about 18 months and to have been annealed about 21 months before the accident took place. [...] " The findings of the Court of inquiry were that the accident was not due to the fault of any particular persons but the fact that the iron of the shackle was not of good quality and was flawed; that the Company had not taken sufficient care in the selection of shackles ; that no special precautions had been taken to ensure that the iron used for making the shackle was of good quality.
